Community Health Assessment
Community Health Assessment:
Jefferson Parish is currently in the process of completing a Community Health Assessment. This process will allow us to examine both quantities and qualitative health data to determine the top health priorities within our parish. The Community Health Assessment will be released in summer 2023.
Our Vision of a Healthy Jefferson:
We envision Jefferson Parish to be a community where everyone has the opportunity, ability and knowledge to make healthy choices; to access quality, equitable, culturally-appropriate and holistic healthcare services; and to benefit from a cohesive and sustainable public health system.
Guiding Principles:
Equity/Inclusion: We believe that everyone should have the opportunity to be healthy and that public health and healthcare services must meet the needs of our entire community.
Collaboration: We believe in a cohesive public health and healthcare system that community members can easily navigate which can only be achieved through collaborative partnerships focused on innovation, coordination and integration of services.
Engagement: We believe that our public health system must engage all members of our community in order to best identify their needs.
Sustainability: We believe in setting goals to develop a robust and sustainable public health system that our community can rely on.
Compassion: We believe that we can only achieve our goals when all people are treated with dignity and respect.
Community Health Improvement Plan:
Once the Community Health Assessment has been released, Jefferson Parish will create working groups to address the top health priorities. These groups will meet every other month for one year to develop our Community Health Improvement Plan. Details on these working groups will be released in summer 2023.
